Who We Are:

Substance Use Disorder Services staff at The Elliot assesses, treats, and supports individuals with SUD who are in or seeking recovery. Our multidisciplinary team includes licensed mental health and substance use clinicians, psychiatric providers, RNs, case managers, and recovery support staff. Our team also works closely with primary care providers to ensure continuity of care.

About the Job:

The MOUD Care Coordinator is responsible for engaging and supporting patients receiving Medication for Opioid Use Disorder Treatment (MOUD) in the Substance Use Disorder Department. Provides skilled nursing services to opioid dependent patients and their significant support systems. Works with the patient and family, provider, and health care team to coordinate care resources and services to improve the quality and effectiveness and decrease the cost of patient care. Demonstrates comprehensive knowledge of community resources and functions as a liaison to facilitate patient access. Collaborates and communicates with stakeholders regarding improvements and the success of the MOUD Program.

What You'll Do:

  • Performs key functions related to the success of the MOUD Program including patient outreach via telephone and office encounters, coordination of patient activities, and the appropriate utilization of services.

  • In collaboration with the clinician performs initial assessment and intake obtaining social, medical, mental health, substance use and medication history.

  • Assesses and monitors patients in the induction, stabilization, and maintenance phases of treatment.

  • Provides ongoing management, education, and support of patients in all phases of treatment including: nursing visits, urine toxicology screens, routine labs and as needed, medication teaching, monitoring, pill counts, observed dosing and medication refills.

  • Coordinates care across the continuum by working to close care gaps. Provides post-discharge follow up calls to hospitalized patients, acts as a liaison with specialty clinics, addresses and works to resolve patient concerns or barriers to treatment plan adherence, and provides a link to appropriate community resources. Facilitates patient access to resources including financial assistance.

  • Collaborates with ACO Case Manager, providers, and other members of the healthcare team in identification of patients needing referral to higher intensity case management services.

  • Aware of and responsive to cultural and demographic diversity of the population.

Who You Are:

Education: Bachelor's degree in nursing required.

Licensure/certification: Current license in the State of NH as a RN required. National Specialty Certification in case management obtained within two years of date of hire.

Experience: Two or more years' experience with the SUD population in a clinical setting, outpatient setting, or care coordination experience is preferred.
